The middle mouse button is set up for "use" If you have nothing to use.. you get a "click" sound..

You no go fast..

To go fast. learn to strafe run.. or play on maps with scout thingy.. you go faster...

bind mwheelup "+button2"

or

bind mwheeldown "+button2"

I used to have mwheeldown +button2 but I unbound in the last month or so and changed it to a relatively hard to reach key.

I got into a bad habit of clicking and clacking all the time and when I was watching a demo from another player's pov vs. me I was able to hear myself clicking and giving my position away.

^I had a similar problem with smilies, though that was more about me annoying the crap out of people than giving away my position. I unbound them completely.
