Game doesn't start properly

Occasionally, I boot a game in practice or online mode and, after the advertisement, I just see the Quake logo with a Quit button below. I am able to click it and quit, but other than that it won't let me do anything. This only happens once in a while, and I can't figure out what's causing it. Any help is appreciated.

In practice mode it can be because you don't have the map you attempt to play on.
You can always open the console and read what's in there; maybe it will give you an idea of the error.
