Quake Live plugin always crashes

Give a try to Qlprism. Just make sure you don't install it to \Program files, but somewhere else.

It's also my personal opinion that it's a little bit bloated now because of the great number of plugins, so I recommend that you disable all the plugins and only enable the ones you really need (for example, chat history and message beep are both very useful imo).
